Registration may be completed
ONLINE.
For your convenience, a Club representative will be at each practice to assist with registration. For players selected by SFE, each player/parent must deliver to the Club representative the following:
1. A check in the amount of $400.00 (350.00 if paid by July 15th, 2009) for each player; and
2. A fully completed and executed (with each page initialed) Player/Parent Contract; and
3. A fully completed and executed Player Registration Form; and
4. A fully completed and executed Medical Release Form; and
5. A fully completed and executed Parent Consent Regarding Use of Photograph and Name; and
6. Four (4) passport pictures; and
7. Copy of each player’s birth certificate or passport.
